Welcome to Montville Public Schools. 

We are excited to welcome you and your family into our school community. Please review the information below to prepare for the student enrollment process.
 

1. STEP ONE

Please gather the following documents to prepare for registration:
  • Three Proofs of Residency as described below
  • Birth Certificate
  • Physical and Immunizations
  • Copy of your license
  • Health forms are required by each school. Please follow this link for fillable Health Forms or to download and print them. 
Parents will have an opportunity to upload these forms and documents in our registration portal. Parents may also email or deliver them to school.

2. STEP TWO

Once you have gathered the required documents, you may begin the online enrollment process. This process will take between 30 and 45 minutes; you can save your progress and return to the enrollment at any time. Once complete, be sure to click submit. If changes need to be made after you submit your information, please contact your school directly.  If you need computer access, computers are available at each school. Please contact any school office to make an appointment to use a computer. You may also request paper copies of registration paperwork.

3. AFTER SUBMISSION

Once you have submitted the online enrollment application, the school will contact you to finalize registration. Only a parent or legal guardian may enroll any minor student. The following documents will be required to enroll your child(ren):

You will have the opportunity to upload the required documents during the online registration process. If you are unable to upload, you may make an appointment to bring them to the school. All originals will be returned to the parent/guardian. If you are missing any of the required documents listed, your child’s enrollment will not be complete and an appointment will be required to finalize the enrollment.
 
Proof of Age
  • Official original birth certificate is recommended. If an original birth certificate is not available, other examples of documents which may be used to verify the age of a student are: a photocopy of a birth certificate, earlier school records, state-issued identification document, passport, parent’s affidavit or unsworn statement to a student’s age, a physician’s certificate verifying a student’s age or immunization records.
 
Proof of Residency
 
1st (required)
  • a current signed lease/rental, recent mortgage statement, or sales agreement/deed.
 
2nd & 3rd (required - choose 2 from the list below - all must include a guardian's name and a current Montville address)
  • Parent's valid Connecticut driver’s license/State ID (will accept the change of address sticker on back).
  • Two (2) of the following statements with parent's name and current Montville address: current utility/water/sewer/bank statement. *Note: these 2 statements can only be used as one proof.
  • Current Montville town tax bill.
  • Current vehicle registration or vehicle/homeowner's/renter's insurance.
  • Current pay stub/letter from employer.
  • Current proof of government benefits (Disability, Medicare, Supplemental Assistance Program SNAP) or other government correspondence showing a Montville address.

No student will be permitted to enter Montville Public Schools until evidence of adequate immunization/physical is provided.

3. IF YOU NEED MORE INFORMATION

If you wish to visit the school and/or speak with a school counselor about your student’s schedule, you may do so only after you have finalized enrollment. Please call the school your student will attend to schedule an appointment. No appointments will be scheduled with or at your student’s school until enrollment has been finalized. Due to security, if you do not have an appointment, you will not be granted access to the school building. Below is a list of school registration secretaries.

Integrated Preschool Enrollment Process – For ACCEPTED Students Only
 
Enrollment in the Integrated Preschool Program at Montville Schools is by invitation only.
 
The program is part of our continuum of services for students ages 3 and 4 for whom special education services are required and for peer role models who have applied and been accepted. If your student is transferring from another district and previously qualified for preschool services through an IEP, you should continue with the online registration process outlined below.
 
Peer role models are chosen via an application and lottery process on an annual basis. If your student has been accepted as a peer role model you should complete the registration process.

Please note that this program does not offer open enrollment.

1. ENROLL ONLINE

Once your child has been accepted, please complete registration online. If you need computer access, computers are available at each school by calling and scheduling an appointment.

2. CONTACT OAKDALE SCHOOL TO FINALIZE ENROLLMENT

Once you have completed the online enrollment process and submitted it, you will be contacted by Montville Integrated Preschool staff to schedule an appointment to finalize enrollment. Only a parent or legal guardian may enroll any minor student. The following documents will be required to enroll your child(ren):

Frequently Asked Questions about Enrolling a Student

I already have other children attending Montville Public Schools. Do I need to complete this enrollment application online and prove residency again?
 
Yes, for each new student entering the Montville Public Schools the online application must be completed and proof of residency, student age & health/immunization documents must be provided.
 
I am homeless and I am living in a shelter or doubled up with a family member or friend.
 
Please follow the link to the district policy on homeless students. A school district must immediately enroll a homeless student and allow the student to attend school even if the student is unable to produce records normally required for enrollment, such as previous academic records, proof of residency, or other documentation. The district will then immediately contact the last school attended to obtain any relevant records and make appropriate referrals for required immunizations and health exams.
I have just moved to Montville and have not yet received utility/water/sewer/bank/renter's or homeowner's insurance statements yet. How will I be able to provide these as a supplemental proof of residency?
 
Most utility companies will provide a verification letter stating that an account has been opened in your name at your new Montville address. This letter could be accepted as your utility bill requirement. If you can access your statement or account verification online this could also be accepted. Please remember 2 statements can only be used as one proof of residency.
 
I have just moved to Montville and my driver's license/state ID doesn't have my new Montville address on the front. How will I be able to provide this as a supplemental proof of residency?
 
If you go to our local Montville Police Department with proof you have moved (closing disclosure, lease, etc.) they will provide you the sticker for the back of your driver's license/state ID. This will allow you to add your new Montville address to the back of your license/ID. This would be accepted.
 
My child has not had his/her 5 year physical yet. Can I still enroll my child?
 
The enrollment/registration process can be completed without the physical form. This form can be brought directly to your child's school at a later date. Please keep in mind that your child cannot begin/attend school/classes until this form has been received and verified by the school nurse.

My children and I live with a relative who is a Montville resident. What additional documents do I need to bring to my appointment to prove residency?
 
Please click on the link below to download the Statement of Residency for Living with Family Member/Other. This form must be completed and notarized.

The Montville resident that you are living with would need to attend the registration appointment & provide the following items:

I am moving within the Montville School District and need to update my child's/children's busing information. What should I do?
 
If you have a child enrolled in the Montville Public Schools and are moving within the town you will need to have your residency updated. After your updated residency has been finalized, transportation will be contacted to update your student’s bus stop. Please note that bus changes due to updating of residency can take up to 48 hours after notification of the residency change.
 
You will need to provide updated Proof of Residency from the list below & complete a Statement of Residency

I am moving out of the Montville Public Schools District. What do I need to do?
 
If you are moving or withdrawing your student from the Montville Public Schools district, you will need to complete a Student Withdrawal form and submit it to your child's school.

What are the health requirements for a student to attend school?
 
All students entering the school system are required to have the following immunizations:
  • DTaP: At least 4 doses. The last dose must be given on or after the 4th birthday.
  • Polio: At least 3 doses. The last dose must be given on or after the 4th birthday.
  • MMR: 2 doses separated by at least 28 days, first dose on or after the 1st birthday.
  • Hepatitis B: At least 3 doses, last dose given on or after 24 weeks of age.
  • Varicella: 2 doses separated by at least 3 months, first dose on or after the 1st birthday, OR verification of the disease.
  • Hib: 1 dose on or after 1st birthday for children less than 5 years old.
  • Pneumococcal: 1 dose on or after 1st birthday for children less than 5 years old.
  • Hepatitis A: 2 doses given 6 calendar months apart, first dose on or after the 1st birthday.
 
All new entering students must provide proof of a physical within one year of the date of entry.
