Upon registration, you will be asked to furnish one or more of the following items as a documentation of residency:

  • Copy of a valid lease/mortgage agreement/deed
  • Copy of a current utility bill with your address on it (electricity, cable, etc. - Phone bill is not accepted.)

Potassium Iodide (KI) authorization form
